Changed all requires.
Also removed not used json/renderartefact.php

- serve_file now will allow overriding the mime type sent in content-type header
- you can also override the application/inline selector by setting force-download
- added swf to the filetypes.xml list and bumped the version.

Used in the artefact chooser currently to show little thumbnails, but could be used elsewhere, in particular in the 'my files' section.

downloading in IE, totally untested

permissions. Still need to do mime type limiting as part of #441 but
for now the permissions are fixed

and theme set up, parameter cleaning. This  is just being committed now
so I can pull, not to be considered complete
