back to pieform configure function, fixed up param_retrieve to not
use REQUEST (as it reads cookies), but instead to read post and get
directly. added contextual help for login box

and add some description for it as well

sure that the formatting remained reasonably consistent between switching
from the WYSIWYG editor to the textarea and back

duplicating the html.  this means page help works too
