Removed inner-link class from the template because it is unnecessary, inner-link class has higher z-index which make it sit on top of the link that toggles the collapsible.

- Move style.css file in plan plugintype to sass features folder since
  the plugin is part of Mahara core
- Add classes in task related templates for future styling
- Use task-status class to apply some styles that will force task-status
  cell to be only as wide as it needs to be
- Remove styles that are not used

In bug 1438894 the fields for the introduction
artefact were changed.
Now we need to use the description of the artefact
instead of the title to show the introduction text.

Removing update hook to prevent failing upgrades from versions older
than 16.10. It's safe as the next hook will recreate a table and 
resync file types.

We were manually verifying the "hash" JSON field
in external/composer.lock, but this was removed
in Composer 1.3.0
