To allow us to easily turn of a 'bad' auth when it is trying to
connect to a third party server but that server is not reachable.

Currently it makes logging in slow as it needs to timeout on that auth
instance before trying the next one.

If the site doesn't yet have any plugins installed that define webservice
connections, then the institution connection manager screen can't be used.
In which case, we should hide the link to avoid confusing users.

Currently the notification message is sent out in the language of the
institution one is joining if that institution has language set. But
if the user being added to the institution has a language preference
set then it should be respected.

Institution names were not being escaped properly in the
accesslist.

This patch escapes them properly as well as clearing the
compiled cache for the templates where this problem occurs.

Also removed link to institution page for "No institution" (ie. the institution

When the staff statistics access option has been
turned off,the institution statistics display message has been
made clearer letting the user know they have no access.

Also fixed problem where user is admin in one institution and staff
in another but can not see stats for the institution they are a staff
member in (when staff statistics are turned on)

Bug 1264429: This patch creates the table, refactors the get_institution_config() method,
the Institution class, and the institution editing page, to use the new table.

Henceforth columns should only be added to the main "institution" table if they represent
a required setting (like name and displayname), or they need to be accessed frequently by
direct SQL queries.

Bug #1162134. Previously we were storing the "institution default"
value as "-" in the database. I've changed this to saving a constant,
LICENSE_INSTITUTION_DEFAULT, which is defined as "(institution default)".

When arriving at Mahara for the first time via SSO, the newly created user
is added to an institution.  During that process an attempt is made to add
institution access to the user's profile view before it gets created.  This
patch adds a check (and when the profile view eventually gets created,
institution access is added for all the owner's institutions).
