bug #605747 and bug #806501

When a user uploads a new profile icon, it now gets added to the
"images" folder in their files area. Profile icons are emphasised and
descriptions aren't editable. Warning notifications, when deleting a
default profile icon or when deleting a folder containing profile
icons, are implemented.

* success function (FileBrowser javascript class) is renamed to "callback" as it
  will be used to process errors as well.
* upload_success is renamed to upload_feedback.
* ArtefactTypeFileBase::files_form missed jserrorcallback definition.

    * Choosing of either image inserting method disables another one.
    * When editing existing image, the correct method is used (ether URL
      substituted or image choosen from dropdown list)
    * Image description (alt) is taken from attached image description if one
      is avialable.

When adding blogpost attachments, upload & browse defaults to the home folder instead of the 'blogfiles' folder

Use the $title variable we prepared earlier for the page title, which silences a warning when visiting the new post page.

* Set the page title within this script, so we can say which blog we are making a new post for.
* Removed the description for the blog post title, it's a bit condescending.
* Display tags on an attachment properly.

The name is largely irrelevant as it's only the creator of the file.
