The standards for the teaching profession have changed
in Aotearoa New Zealand. Therefore, the old PTC
framework is removed and the new standard matrix
uploaded. The te reo Māori framework is added as
well. That way we can illustrate what a non-English
framework looks like.

Added an upgrade path so that one can add the new frameworks to
existing sites

 - Modified tags in template so screen readers
can see them correctly
 - Tabbing goes through each cell of the table
and collapsible sections heading
 - Opening a modal sets focus on the delete button
of the modal ('X')
 - Tabbing inside a modal dialog loops through the
elements of that dialog
 - Closing modal dialog sets focus back on the
annotation cell
 - Clicking on standard title (first cell of a row)
will display description of standard. The element
can also be reached with tab key and description
can be opened with enter key. The description box
disappears when it loses focus

If we create a SmartEvidence collection and add a page to it then
turn SmartEvidence off for the institution and try to go to the matrix
page, we get an error page with link to first view in collection.

If the collection has no pages though the matrix error page has a link
going nowhere.

An earlier rendition of the way we added new matrix files was to place
them into the matrices dir and then upload them by name only.

But now we use the file upload field and so don't need this check
anymore

This patch:
 - removes the need to set priority value in .matrix file
   they are worked out now based on the array generated
 - changes the way you indicate parent/child relationships by adding a
   'uniqueid' to the parent and change 'parent' value for its children
   to the value of the uniqueid
 - stopped an element showing the dot on the matrix if it has children
 - added indentation for the name (1st) column on matrix page so we
   can see how things relate to each other better

When you click on the hollow circle (rather than dot) on the matrix
screen you will get an annotation feedback form in the modal dock
rather than the create annotation form.

So that we can show only a portion of the pages and use a
'next'/'prev' toggle to scroll through them.

This works by hiding/showing columns based on the td.index number.
