This was only causing noise, there was no breakage caused because of it.

Correct the query for obtaining the institution list in the case where there are deleted institution staff and administrators.

Show the name of the current default theme on the 'site default' option in the dropdown for institution theme.

This involves making sure that the lastauthinstance field of the usr table doesn't refer to the auth instance too.

Don't bother showing the authtypes element if we are creating a new institution - it will only confuse people. Make the length of the 'maximum number of users' input shorter.

The name is largely irrelevant as it's only the creator of the file.

Put the 'locked fields' in a fieldset when adding/editing an institution. This shortens the default length of the institution form quite considerably.
