When a collection is submitted to a group and then
the group is deleted, then the collections are
released.

Also, update mp4_video type to mp4 in the internalmedia block
configuration settings.

And, remove the id column from the artefact_file_mime_types table. It's
not required with the addition of the primary key made up of the
mimetype and description columns.

Where a user has multiple journals but has account preference for
multiple blogs turned off.

To test: Add multiple blogs to a user then in db set the user
preference for multiple blogs to off. Logout and login again as the
user and go to the journals page.

Institutional admins should not have the "Contact us",
"Repeat virus upload" and "Virus flag release" fields available
in the Activity Preferences Notification settings.

They should have "Objectionable content" field available.

The sql queries were missing the COUNT() parameter, but it turns out
we should just use record_exists_select in this case as we don't care
how many exist but if they do or not.

Fixed error when

accessing page webservice/testclient.php
that showed a debug message "Only variables should
be passed by reference".

importing Leap2A file that showed same message.

When an anonymous comment was left it would set up a watchlist for the
view to sent to root user (user id = 0)

This patch stps that happening and removes any existing rows in
usr_watchlist_view for usr = 0
